With the new year now upon us, your insurance company, physicians, or residential address might change. We are asking all parents and caregivers to please provide updated information upon check-in at your appointments. If you can please have any new insurance cards, ID or proof of new residency ready so we can properly update your records. This will provide a smooth transition for you and your family in future scheduling, explanation of benefits and billing. Thank you sincerely from Elite Therapy Center admin staff!
